Sermorelin Disclosure, Assumption of Risk & Waiver

This Sermorelin Disclosure, Assumption of Risk & Waiver (“Waiver”) applies to all sermorelin-related information, assessments, consultations, recommendations, or services made available through Rejuvimen LLC, including its DBAs Lavena, Lavena Well, and Lavena Wellness (collectively, the “Company,” “we,” “us,” or “our”).
By accessing sermorelin-related content, completing an intake, or proceeding with any sermorelin-related consultation or treatment, you expressly acknowledge, understand, and agree to the following:
1. NON-EMERGENCY TELEHEALTH SERVICES
Sermorelin-related services provided through this platform are non-emergency in nature and may be delivered via telehealth. Telehealth consultations do not include an in-person physical examination and may limit a provider’s ability to identify certain medical conditions.
If you experience chest pain, shortness of breath, fainting, severe dizziness, or any life-threatening symptoms, call 911 immediately.
2. NO PRACTICE OF MEDICINE BY COMPANY
You acknowledge and agree that:
The Company does not practice medicine
The Company does not diagnose, treat, prescribe, or administer sermorelin
The Company does not manufacture, compound, distribute, or dispense medications
No doctor-patient relationship is created between you and the Company
All medical decisions are made solely by independent, third-party licensed healthcare providers exercising their own professional judgment.
3. OFF-LABEL & FDA STATUS DISCLOSURE
You understand and acknowledge that:
Sermorelin is commonly prescribed off-label
Sermorelin is not FDA-approved for anti-aging, wellness, hormone optimization, muscle building, fat loss, or longevity purposes
The FDA has not reviewed or approved sermorelin for safety, effectiveness, or quality for these uses
Off-label use is legally permitted but involves inherent uncertainties and risks.
4. LIMITED SCIENTIFIC EVIDENCE
You acknowledge that:
Scientific evidence supporting sermorelin therapy may be limited, evolving, conflicting, or preliminary
Long-term safety data may be unknown or unavailable
Outcomes are unpredictable and vary significantly between individuals
No statements made by the Company constitute a promise, guarantee, or representation of benefit.
5. COMPOUNDED MEDICATION DISCLOSURE (IF APPLICABLE)
If a licensed provider independently prescribes compounded sermorelin, you acknowledge that:
Compounded medications are not FDA-approved
They have not undergone FDA premarket review
Variability in formulation, concentration, and purity may exist
Compounding pharmacies are regulated at the state level
6. POTENTIAL RISKS & SIDE EFFECTS
You acknowledge that sermorelin therapy may involve known and unknown risks, including but not limited to:
Injection-site reactions (pain, swelling, redness)
Headache, dizziness, nausea, or flushing
Fatigue or sleep disturbances
Changes in blood sugar or insulin sensitivity
Hormonal or metabolic effects
Worsening of existing medical conditions
Serious injury or other unanticipated adverse events
7. ASSUMPTION OF RISK
You knowingly, voluntarily, and irrevocably assume all risks, whether known or unknown, foreseeable or unforeseeable, associated with sermorelin therapy, including risks related to:

Lack of benefit or adverse outcomes may occur
Any references to potential benefits are not guarantees
9. USER RESPONSIBILITIES
You agree to:
Provide complete and accurate medical information
Disclose all medications, supplements, and health conditions
Follow provider instructions exactly
Seek in-person or emergency care when appropriate
10. RELEASE OF LIABILITY & WAIVER OF CLAIMS
To the maximum extent permitted by law, you irrevocably release, waive, discharge, and hold harmless the Company from any and all claims, damages, or causes of action arising from or related to:
Sermorelin therapy or lack of results
Off-label or compounded use
Provider clinical decisions
Adverse reactions or complications
This waiver includes claims based on negligence, to the fullest extent allowed by law.
11. INDEMNIFICATION
You agree to indemnify and defend the Company against any claims, losses, damages, or expenses (including attorneys’ fees) arising from:
Your use or misuse of sermorelin
Failure to follow medical guidance
Inaccurate or incomplete information you provide
12. ACKNOWLEDGMENT & BINDING CONSENT
By proceeding, you affirm that:
